Transitional Care Liaison-RN Primary Job Function

Clinical Liaison-RN has the responsibility for on-site assessments following inquiry and increasing quality by representing Creekside Health and Rehabilitation to the Medical Community.

Responsibilities

Clinical Liaison Primary Responsibilities

 

The Clinical Liaison-RN performs assessments for admissions as well as develope and maintain visibility in the community. This includes:

  • Use clinical judgement to assess resident for admission
  • Follow up on all inquiries to answer further questions in the local area
  • Develop new medical markets outside of local area
  • Develop new and maintain current Medical referrals sources
  • Perpetuate the facility's high visibility in the medical community to project an image as a senior care resource.
  • Keep DCPs & other referrals sources aware of services offered and bed availability.
  • Plan and coordinate "luncheons" and other events with medical referral sources
  • Staying aware of the competition and coordinating the marketing plan in conjunction with the A/C and Community Liaison.
  • Coordinate assessments, approvals/denials between facility and referral source.
  • Work with Liaison to develop relationships and partners with insurance ACO/Bundle payment providers.
  • Completing admission paperwork when possible while patient is in hospital.

Qualifications

Clinical Liaison-RN Qualification/Requirements:

  • Must possess specific educational and experience requirements such as:
    • An Registered Nurse (RN) license with the State of Indiana
    • Knowledge on the use of personal computers, e-mail accounts, software programs such as Word and Excel and minor office machines such as faxes and copiers;
